Office Hours 

 

Monday - Thursday 8am - 4pm CST. Please note, Kuponya's office hours differ from available appointment times.

Communication

 

Often, because we are assisting other clients, we are not immediately available by telephone. At these times, you may leave a message and your call will be returned within 2 business days. You may also communicate via the patient portal.

 

If you are experiencing what you believe is a negative reaction to your medication, having suicidal or homicidal thoughts, or feel unsafe, please call 911 or go to your nearest emergency room for evaluation. 

Appointments

Serving clients, ages 18 - 65, residing in the following states, only:

  • Maryland

  • Nevada

  • Oregon

  • Texas

  • Washington, D.C.

  • Washington state

All appointments (regardless of which option you choose) are conducted utilizing a secure video platform. This requires that you have access to a webcam.

Please note, we do not provide Talkspace services to clients residing in Texas. Those residents must click the link above, to make a telehealth/video appointment.

To determine if our practice is appropriate for your needs, you must complete the short new client evaluation.  You will be contacted within one business day, regarding that decision. We will require pertinent medical records, as well as any additional forms you are asked to complete, prior to your first appointment. Specifics will be discussed on a case by case basis. 

 

*You must have a primary care provider throughout the duration of your care.*

 

When it's time to "attend" your appointment, simply "check-in" using the link on the top right corner of this page. You will then be taken to a HIPAA compliant site for our video appointment. (It's encouraged that you check in early to give yourself plenty of time to test your video and audio connection, and to avoid needing to reschedule.)

 

Insurance

If our provider is not "in network" with your insurance carrier, you will be responsible to pay out of pocket. However, at the conclusion of the visit, you will be sent a visit summary that includes billed charges. You can send this "superbill" to your insurance company for possible reimbursement. The amount will be determined by your insurance carrier, and they can walk you through their required steps.

*We do not accept Medicaid, Medicare or Medicare Advantage Plans*

Visit Fees

Fees/copays will vary for those with insurance, per your individual policy.

 

For those without insurance, and choosing to pay out of pocket:

 

  • Initial evaluation $ 250 

  • Follow up visit    $ 125 

 

All fees, including copays, are paid in advance, through our website. Visa, Mastercard, American Express, Flexible Spending Account (FSA) and Health Savings Account (HSA) are accepted. (Please note that the card must have a Visa or Mastercard logo.) We do not accept mailed payments. If payment has not been made at least 24 hours before your appointment, it’s equivalent to a nonconfirmed appointmentand your appointment will be canceled. 

 

For those needing interpretation services, a third-party interpreter is required, at your expense. Friends and/or family members are not acceptable. The video conferencing platform used allows for three-way conversation, so your interpreter does not need to be at your location. 

Missed or Canceled Appointments 

 

Appointment reminders are sent via email. Cancellations must be made at least 24 hours prior to the scheduled appointment, and can be done several ways (self-cancel via patient portal, send message requesting cancellation through the patient portal, or voice mail message to cancel). Because payment is expected in advance, if you cancel at least 24 hours prior to your appointment, you will get a full refund. If you cancel within 24 hours of your appointment, you will not be refunded fees paid. 

 

Examples:

 

  • If your appointment is Wednesday at 10am, you must cancel before 10 am Tuesday. 

 

  • If your appointment is Monday at 2 pm, you must cancel before 2 pm Sunday. 

 

We understand things happen and you may miss your appointment for reasons that are out of your control, so we'll issue a refund. If there are 3 cancellations within 24 hours, you will be discharged from the practice. 

Late Policy 

If you are more than 10 minutes late, your appointment will be canceled and you will not receive a refund. With an online platform that takes away the stress of weather conditions, traffic and trying to find parking, it is very convenient for you to "attend" your appointments on time.

Medications

Electronic prescriptions are sent directly to your pharmacy within your state of residence. Prescriptions are not mailed. You will receive enough medication refills until the next scheduled follow-up appointment, therefore, sending a message about refills is generally not necessary. However, occasionally refills do not always get sent to pharmacies as planned. In this case, please place your refill request through the patient portal.  If you miss an appointment that was meant to discuss your progress and refills, your refill will be sent just once, and for a $40 fee that must be paid in advance. Changes are not made to your medication(s) without an appointment.

A $25.00 fee is charged for re-sending prescriptions to a different pharmacy, after your appointment. Please provide any new pharmacy information at the time of the appointment to avoid this fee.

Often times we receive duplicate refill requests from pharmacies, which interferes with productivity during the clinic day. To address this issue, we simply do not accept or respond to faxed refill requests. 

*We do NOT prescribe controlled substances (Ativan, Alprazolam, Clonazepam, Adderall, Vyvanse, etc.)*

Administrative Policies 

  • FMLA - Family and Medical Leave Act (FMLA) paperwork in the mental health outpatient setting is uncommon because outpatient appointments do not often conflict with work requirements. Clients often return for follow up visits every 1-3 months and those appointments can be scheduled, by the client, around work commitments.  

  • Disability- We do not perform assessments or complete paperwork for disability claims.

  • Service animals- We do not complete paperwork, nor draft letters, requesting approval of service, companion, or emotional support animals. 

  • Legal Testimony- Legal testimony can often be damaging to the relationship between a patient and his/her mental health provider. Because of this, we require that you employ independent forensic psychiatric or psychological services should this type of evaluation or testimony be required.

 

Client Rights: You have the right to receive considerate, safe and respectful care, without discrimination as to race, ethnicity, color, gender, sexual orientation, age, religion, or national origin. You also have the right to ask questions about any aspects of your treatment. While we do hope you will speak with us if you are unhappy with our services, if you prefer to see another mental health provider, you are free to end treatment at any time. 

 

 

Provider Rights: We have the right to terminate our relationship when we believe that our services are no longer beneficial to you and when we feel that another professional would serve you better. We also have the right to terminate our relationship when you do not cooperate with the proposed treatment, and if you or your family member/significant-other are hostile or aggressive.
